Program to treat patients at home unvieled
Christiana Care will take part in the Independence at Home Demonstration Project to treat patients at home.
Senator Tom Carper will help hospital officials unveil details of the pilot program this afternoon. The program is designed to test whether health outcomes can be improved and costs can be lowered for patients living with multiple chronic illnesses. The project starts today and will run through and conclude May 31, 2015.
