Teen missing in Dover, Delaware found safe, Amber Alert canceled

Officials said the fire broke out around 1 a.m. on Tuesday in the 3400 block of Hope Street, and that a 45-year-old woman, a 6-year-old girl and a 5-year-old boy died.

A spokesperson for Gauger Cobbs Middle School said there were a few minor injuries to students, but that no one required hospitalization.

The NTSB said the outdated design of the railcars, combined with SEPTA's maintenance practices, "represents an immediate and unacceptable safety risk."
