Controlling health care costs among Camden’s ‘super-users’

An interesting experiment in the delivery of health care is taking place in Camden, where a team of health care providers is working with chronically-ill patients who are frequent users of hospitals and their emergency rooms. The program run by Camden Coalition of Health Care Providers incorporates the “medical home” mode of care and is directed by physician JEFFREY BRENNER. He joins us, along with nurse practitioner KATHY JACKSON and social worker KELLY CRAIG, to talk about the program and its successes and challenges to date. Their work was featured in a recent New Yorker article by Atul Gawande.
